Follow these instruction:
You may submit the application to the visa office of the Chinese Embassies or Consulates in person.
Or you can trust someone else (e.g. friends, relatives), a travel agency or a visa agency to act on your behalf if you can't come personally. Power of attorney is not required.
Mailed applications are not accepted and will be refused by most of Chinese embassies or consulates in overseas.
There is no need to make an appointment.

Section 1
Please fill in your name as in your passport and separate your first name, last name and middle name. Note: Surname is last name, given name is first name.
Please select your gender.
Please fill in your Chinese name, if applicable. Note: it must be provided, if your place of birth is China.
Please fill in your current nationality as in your passport, such as USA.
Please fill in your other Chinese name, maiden, religious, professional, aliases names if applicable.
Please fill in your former nationality if applicable.
Please fill in your date of birth.
Please fill in the name of your birth place (City, Province/State and country).
Please select the type of your passport with which you are applying for the visa. Most People should check "Regular". Note: if you use other than regular passport such as official passport, please call us first.
Please fill in the number of your passport with which you are applying for the visa.
Please fill in the date of issue of your passport.
Please fill the name of place of issue of your passport. Note: that's what under "Authority" in your passport. If your passport was issued recently in the US, you may simply put down: US Department of State; If your passport was issued overseas, you need to indicate the country where it was issued. For example: US Embassy, Germany.
Please fill in the expiration date of your passport.

Note: multiple entry visas is only issued to US passport holder who have been to China and can provide a photocopy of previous Chinese visa issued in recent 5 years. First time applicant may apply for single or double entry visa.
